GP Chronic Condition Management Plans

GP CARE PLANS / COMPREHENSIVE CARE APPOINTMENTS

Chronic conditions such as diabetes, osteoporosis, arthritis, asthma, cardiovascular disease, mental health conditions and obesity often benefit from regular review and coordinated care. Comprehensive Care Appointments attract a higher Medicare rebate, allowing us to offer longer appointments with a lower out-of-pocket cost than standard consultations.

*Medicare rebates are subject to eligibility and may change over time.

What is included?

  • Review of chronic medical conditions
  • Medication review and optimisation
  • Preventative health screening
  • Lifestyle advice and goal setting
  • Referrals to allied health professionals where appropriate
  • Coordination of care with specialists and other healthcare providers
  • Development or review of your GP Chronic Condition Management Plan

Why are these appointments offered at a reduced gap?

Because Medicare provides additional support for chronic disease management, we are pleased to offer these appointments with a lower out-of-pocket cost than equivalent standard consultations. This allows us to provide the time needed for thoughtful, evidence-based care while maintaining continuity and affordability for patients.

How often should I have one?

Most patients benefit from a Comprehensive Care Appointment every 3-6 months, depending on their medical conditions and treatment goals. Your doctor will discuss an appropriate review schedule with you.

To access these appointments, you need to be registered with Bronte Family Doctors Mymedicare.

*Please ensure when booking that you select the ‘GP Chronic Care Plan’ Appointment type so that our team can help ensure that you meet eligibility requirements prior to your appointment. Standard consultation fees will apply if you are not eligible.*
