Living with a chronic health condition often means managing several aspects of your health at once. Having a regular GP who understands your medical history and coordinates your care can make this much easier.

At Bronte Family Doctors, we provide ongoing care for people living with chronic and complex health conditions, with a focus on keeping you well and helping you achieve practical health goals.

Your consultation may include

  • Review of your current health conditions
  • Weight history and previous weight loss attempts
  • Nutrition and eating patterns
  • Physical activity
  • Sleep and stress
  • Medications that may contribute to weight gain
  • Hormonal factors, including perimenopause and menopause
  • Assessment of cardiovascular and metabolic health

What conditions can we help manage?

Chronic disease management may be helpful for people living with conditions such as:

  • Diabetes
  • Heart disease and high blood pressure
  • Asthma and chronic lung disease
  • Arthritis and osteoporosis
  • Chronic pain
  • Kidney disease
  • Neurological conditions
  • Multiple or complex medical conditions

GP Chronic Condition Management Plans

If you have a chronic medical condition that has been, or is likely to be, present for six months or longer, you may be eligible for a Medicare-funded GP Chronic Condition Management Plan (GPCCMP).

Your GP will work with you to identify your health priorities, develop a personalised plan and coordinate the different parts of your care.

Where appropriate, your plan can also provide access to Medicare-subsidised allied health services, such as physiotherapy, podiatry, dietetics or exercise physiology.

Care that continues over time

Chronic disease management isn’t just about completing a plan. Regular reviews allow us to monitor your progress, review medications and results, update your goals and make sure your care continues to meet your needs.

Our aim is to provide coordinated, practical and proactive care, with your GP helping to bring the different pieces of your healthcare together.
